Unable to open UK MTD Submit Via CSV

Hello,

We have installed the necessary EMEA and UK LOcalization bundles in our sandbox. We get the following error when trying to open the UK MTD Submit Via CSV. The path to get to this is, in Administrator role, Reports ->VAT/GST ->UK MTD Submit Via CSV.

We are not using the SuiteTax and have downloaded the International Tax Reporting bundle to help with this.

The error on the screen capture is, "TypeError: Cannot call method "addField" of undefined (/SuiteBundles/Bundle 43003/src/amd/lib/FormBuilder.js#39).

Any help to resolve this is appreciated.
