Facing error when printing individual statement of a customer.

When selecting » customers » Individual Statement » after landing on print Individual statement screen » I have selected the customer from customer dropdown » and the Form dropdown with the required value and Statement Date,Start date were default set and clicked on the print and getting this below error.Please can anyone let me know,what needs to be done here!
