Fusion OPSS Report, 'Comment' column

Need a 'Comment' column in OPSS report for capturing additional comments

We are currently using OPSS report to identify the list of users granted access and revoked access in Fusion. But then OPSS report is not able to capture any additional comments that the agent would like to add, while granting/revoking access, e.g. ticket number or the agent id.

Can any one suggest, how to get a 'Comments' column/field added to capture additional details in OPSS report?
