Feedback forms in Oracle Knowledge Advanced

How to expose feedback forms(created questions and answers) to end user or agent to give their feedback.

Hi All,

I have created a form with two questions and answers and have saved the form. but couldn't find the way to expose the form  for providing feedback . can anyone help me out in using this form for giving  feedback either by agents or end customers in oracle knowledge Advanced.
