Agent feedback on answers in kb

We use our knowledgebase internally for agents to find info, knowledge and guides/manuals to help them in their daily operations.
But we want agents to give feedback on the articles/answers in order to improve the content so I'm trying to figure out a way to add a survey to the answers.
I have so far managed to create a simple survey with yes/no question and a comment field, and I have also added it to an answer - se screenshot.
