Daily Covid19 self-report checklist: Is it possible with Checklist or HNS Self Service?

Does anyone have any thoughts on the best way to accomplish the following? 

We are going to have a small subset of employees begin returning to the office in about a month. 

We'd like to have them use Oracle HCM Cloud on their laptops or phones each day they are coming into work, to answer a simple series of questions before entering the building. Such as do you have a fever? do you have a chronic cough? have you had any difficulty breathing in the last 48 hours? and so on.
