Just a heads up that if you create a Questionnaire for the Workplace Health and Safety Module, leave the "Description" field on the Questionnaire "Basic Information" configuration page BLANK (not a required field).  I found that if I have a Description on my Questionnaire and select it as my Investigation Questionnaire, it generates the error "Attribute QuestionnaireDescription in view object InvestigationSummary cannot be set."  Once I removed the Description from the Questionnaire, the Error stopped occurring when assigning the questionnaire to an investigation.
