Create incident on survey submission and include responses in Customer Entry

We have a weblink survey and we'd like to have responses recorded as a 'Customer Entry' in an incident (that will be assigned to a queue). I'm struggling to work out how this can be achieved so would appreciate your advice. [Side note: Surveys are not my strong point, obviously!!]
