BUI toast notifications not working - what am I missing?

Hello,

I am trying to run the example code provided in the BUI Extensibility Framework Documentation for a simple toast notification and it does not appear to be working.  The example code in the documentation contained a syntax error and needed some references fixed (my updated code is below).  This seems simple enough and my updated code runs but for some reason I get no notifications.  Is there a profile permission or something that needs to be set to allow toast notifications to work in BUI?

Any feedback would be much appreciated.
