Disposition Input Widget

Hi all,

I have need to ask the user to input a disposition value using a form on the customer portal when creating a new ticket. I can see there is an input widget for the product/category hierarchy but I cant seem to see one for the dispositions. Would I need to create a custom widget and if so is there any step by step guides on doing so?

Since dispositions is the only other built in way to have hierarchy values it would be handy to use for a multitude of things.
