How to add Survey Questions to a Customer Portal page

I have been told by Oracle that I can add Survey questions to a webform (web page) which I can create in customer portal

I know how to create a new web page in customer portal, but I do not know how to add survey questions to it. The idea here is that a person fills out the survey and the results come in as a normal survey from Oracle comes in, then we can report on the results as usual. We can't use the regular survey because we are trying to pass an incidentID to associate the survey results to an incident. This association would normally happen using oracle Transactional surveys, but in our case we cannot use a transactional survey because we
