Self-Identification of Covid vaccination

With the uprise of vaccination distributions and return to office policies being enacted. I am curious to know if any other clients have received requests for using HCM to capture record of employees who have been vaccinated. Exploring if there is a way to capture this via self-service?  Similar to the disability form available. The idea is the form would allow the employees to self-identify and this would be stored in the system with role specific access to the data.  

Has anyone come across this requirement and found a way to implement?
