Creating a custom object record after finishing a guide

Hi All,

There is a requirement where we need to initiate a Guide/Script from a contact record and after finishing the Guide/Script, we have to capture all selected options from the Guide/Script and create a custom object record (child of contact) to store this information. Please let us know if any one has worked on similar kind of requirement.
