Escalation isn't triggering for new rule state

I have a two level escalation for Answers. The first level escalates fine. But the second one will not.

Content (required):

I am trying to set up escalation rules for Answers where after so much time a review escalation level is invoked and the answer state and status change to Review. Then after a period of time in Review, if the Answer hasn't been updated, it is to be escalated and status set to Unpublished.

I have turned on versioning and am using the Review On field to automatically set the status to Review and using that information to escalate the Answer to In-Review and change the State from Answers In-Use to Answers In-Review. This all works fine.
