OCI - OSMS Update Stuck at 0% for Many Hours

Symptoms
OSMS upgrade triggered from the console stuck at 0% for many hours.
Cause
The Upgrade request is received by the instance OSMS Plugin but OSMS process is in defunct state and it affects the invocation of update on the client.
Example: bind package upgrade is received in OSMS plugin.
