We've been trying to figure out if there is a way to send something along the lines of a questionnaire or survey, or even just a form, to contacts and have those responses tethered to their contact record (if possible).

I know that it's possible to send a survey to a large group of people and then view responses by contact. This doesn't quite work for our purposes. We need something more along the lines of a standard form or set of forms that we can build, and which we can distribute to contacts on as needed basis. It might be one recipient today, one next week, three the week after that, etc in perpetuity. Ideally, there'd be a link from the contact record to their form/questionnaire responses.
