How to update Effective Start Date (HSDL)-how to find LOV for Information type, Information Category

Query: I am trying to update "Effective Start date" thru HSDL with below template I am not clear about value in below column, have tried various combinations by seeing articles on CCC, could you please help me what would be correct values for highlighted column?

Solution required: Please provide any guidance/articles as to how to find right LOV for highlighted attributes or any other HSDL attributes.
