Multiple FileAttachmentUpload2 on a Form in CP2

Hi,

I have added two "FileAttachmentUpload2" widget on a ask.php file. But i am getting the error (The specified file path could not be found). After that, I had created a custom widget (hideShowFileAttachmentUpload2) for another file upload field, but I am getting the same error. 

Could some body help regarding this.
