I'm creating my own thread for this, because it seems my requirements differ slightly from other solutions explained.

Basic rundown:

I have a form page that passes values directly into RightNow's mapped custom fields:

The fields are First Name, Last Name, Email Address, and 2 additional product selection fields. 

So far, so good.  This part has been created and works fine.  Now here's the tricky part:

I have 5-7 additional input and/or option fields that need to display (pass values to) within the 'Private Notes' area.

My first question:  Is this even possible? Can this be done with "1" form?
