Transaction Survey not going out

We created a post satisfaction survey in Survey Explorer and have it launched. Then we created a new rule for incident object where on status change to Solved, the survey needs to be sent out to the contact. We used out-of-box "Send Transaction Survey" option to do this but the survey is not going out.
Can someone please let me know if I am missing something in this configuration?
