Add-In drops off Workspace

Hi team,
I have an Add-In that each time I edit the workspace, regardless that I may not touch the Add-In, eg add a new field or change a label, when it is saved the Add-In looks to still be there but no longer functions.
I need to ensure that each time I make any edits to the workspace, I remove the Add-In and then re-insert back again before saving it.
Any suggestions or ideas what may be causing this?
Currently using Feb 14 version.
