Post-Chat Survey not Showing Contact Information

Good Day Everybody,

We are doing a post-chat popup survey and all that works fine.  We have verified that the survey and the chat are linked but when I view the results of the survey it shows blank First Name / Last Name / email address and I know that information is showing on the chat and the survey is therefore not linking to the contact.  This worked twice previously and the survey hasn't been changed (still developing the details of the chat deployment) but I need to figure out why the survey is keeping the contact information.
