Custom Survey Not Appearing in Completed Surveys Reports

Hello Dears,

I have created a survey entirely using an HTML template and designed to look similar to a matrix survey (i.e., a series of questions with radio choice answers). The radio buttons are clickable on the survey page but what I noticed is that once the contact completes the survey and presses the "Submit" button, the survey is not validated as complete in Oracle. Additionally, we don't have a scoring criteria for our answers choices so we have no way to compute a total score from this custom survey.

Is there a way to add scores per question in this particular case and make the survey appear as completed in our survey reports? (I'm assuming the computation of the total survey score is related to the survey being marked complete upon pressing the "Submit" button?)
