Create Incident through website link survey

Hi,

I create a website link survey that pop up after closing a chat session, it contains 3 questions that if one of these questions are answered badly i create an incident, but once its answered no incident is generated.

Someone know if its possible to create an incident in this type of survey ?

Actually i send surveys through transactional survey and had no problem generating incident.
