Anybody have a good polling survey example?

We recently purchased the Feedback component with the intent of using some transactional surveys to gather feedback from customers on recent interactions. However, I also learned about Polling Widgets from our Customer Success Manager and I think they might be helpful for our Product Management team as they are rolling out a new version of our web app to customers - they would love to get feedback on their most recent release. I'd like to show them some examples of polling widgets... does anyone in the Community use these and if so, could you help me find your site and the widget
