Website Link Survey and Incident Reference Number

We have a Website Link Survey that a Contact can click on after an Incident is submitted through the Ask A Question page.

This seems like an easy question that I'm overlooking, but what table/field do you use to get the Incident Reference Number (or any Incident details) for a Website Link Survey? I've looked at some of the "canned" survey reports but don't get any results.
