Embed/Inline Survey + checkbox questions

Hey everyone!

We are creating an NPS survey and we want to make it embed in the survey email.

I was trying to find a way to do it, but got stuck.

We have 2 questions: 1st question - "Choice" from 1 to 10 and 2nd question - "Text".

We also want to add a checkbox with 1 question. We want a customer to be able to check it if they wish to be contacted.

I found a few posts on similar topics, but nothing that actually answered my questions. 

Did anyone encounter similar problems?
