Notes On Task Object on Customer Portal

Dear Experts,
I created the new task from customer portal means fill the task form CP & after submit task should save into Agent desktop against task object. I face issue to save Notes field. Also i tried connect php api static code but it's not working. Can anybody please help me. Below is the static connect php api code.
