Image not loading properly in CP using file attachment

Hi everyone,

While creating a custom object, I have checked the file attachment section(screenshot attached). This file attachment section is used to upload the image from the local system to the created custom object. If i am trying to view the image in CP, Sometime the image will be displayed sometimes not. I thought it may be the network issue. But even in good network also the same issue is facing.

Did any one of you guys faced this kind of issue? If you know please give me some advice. 

The code below will store the image in the created custom object
