Create List of Article Feedback Details

When an article is viewed, the user has the option of providing a y/n response to the helpfulness of the article and they are able to provide feedback. I would like to be able to see a list of the feedback provided with the article ID and if possible who submitted the feedback. It would great if I could run a report to give me higher level detail such as the total responses by user, but just a list of responses would be helpful. As is, I can only see this information by pulling up an article in authoring. This is
