Attestation of Compliance for PCI DSS

We are implementing some new systems in our company, and are trying to ensure they are compliant with PCI DSS standards.  We are using another company to assist us with getting all of our ducks in a row.  One of the things they say we require is official documentation from all the systems that house data we use.  One of the documents our representatives are requiring is an Attestation of Compliance from Oracle Service Cloud (see https://www.pcisecuritystandards.org/documents/PCI-DSS-v3_2-AOC-ServiceProviders.docx?agreement=true&usg=AOvVaw2_vh_TCwGFO5guP52AMnKD for a blank example). 

Does anyone know where I could find this document?  Is this something that is available to view publicly, or must it be obtained via unique request?  It seems this may be something specific to each customer, but I can see the possibility of a blanket document that covers all possible applications.
