Implementing Disciplinary Action on OSC

I believe some of experts here, have implemented Disciplinary Action (DA) system for organizations. I need guidance about DA implementation in OSV. That is,
- What flow it should have
- What statuses I should maintained
- What sort of customization would require in this
- Effort and timeline
Client asked to proposed them the flow and statuses etc. For statuses I thought below so far:
- Opened
- Under Investigation
- SC Sent
- Awaiting SC Response
- Awaiting for Decision
- Closed
SC --> Show Case notice
I identified around 20-25 fields which I would create. Is it recommend to create that much custom fields on Incident object? And for the flow, currently I am not certain about. Please provide your guidance on this.
