Skinning of ClickToCall page

Hi All,

 

I have a quick query related to skinning of ClickToCall screen. In the attached screen, we have Country, Phone Number of the user and few text(header/footer).

 

Is it possible to skin this page to get the look and feel of ThirdParty site?

Also add few more fields like Name, Email to the UI and pass few hidden fields like ServiceType and language?

 

If possible, Can you please provide the steps/document link to refer so that we can customize the screen and change the look and feel of it?
