Deployed incident system attribute custom object but do not see it under Relationship Items

I created a created a incident system attribute custom object in order to create relationship to fields for a report. I deployed the object but do not see it under Instert Control > Relationship Items.  I am not sure what I am doing wrong to get this object to display for use.
