Overall Rating isn't shown for user's Performance Document

This is probably an easy fix and I'm just overlooking something small but a client contacted me with an issue for one of their employee's.  Her annual performance document lists a rating for the Goals Section as well as the Competency Section but does not list an Overall Rating.  

The Performance Template and Process Flow are fine as she's the only employee that has had this issue.

Anyone have an idea on the fix here?
