Feedback for an Agent

Hi Fellow members,

I am trying to connect feedback for an Agent. From OOTB Survey feedback, you can provide feedback for Organization level and also there is Survey by Proxy which holds good on a call.

If I send out a link for Feedback to Customer and customer responds - can I attach it to Assigned Agent?

I have tested this by using Custom Objects and Custom fields nothing seems to work and more over it is hard to pull in Agent details in Survey(or I do not know how to pull it, may be!!)

Has anyone done this in real time? Please shed some light on this.
