Certainly - that is a very common way of doing things.
which requires the apex download to be unzipped on the middle tier machine with the apex listener, right?

Right, you need to put the images (and other static contents for APEX) to a filesystem location that can be accessed by the web server. That's the same when using Oracle HTTP Server (OHS) or the Embedded PL/SQL Gateway (EPG). Of course, EPG is hosted by the database internal XDB HTTP Server, so the files have to be uploaded into the XDB in that case.
