So what happens if you have a 4 CPU server and you run it off of the 100 volts versus the 200 volts that are recommended? What is the risk/reward with that scenario?

Well a customer did try it and it seems to have worked for them? The question is if there is something to having the 4 cpu's that requires the additional power vs the 2 cpu setup?
They are trying to check and see if there is anything that is not working correctly because its running of less power than what is recommended, but so far they have not found anything?
My thought is to have them run explorer and let's see if we can find anything there. One with 100V and a second with 200V power.
I know, was looking to get some insight into reasons for different voltage levels. I wasn't sure if it was definitely tied to some power need for a specific hardware piece or something?
In the end if we recommend the 200V then that is what should be used. Not really a break/fix type scenario to begin with, more of a question of curiosity than a real issue.
You might consider going back to them and reviewing basic electrical arithmetic with them.
Each CPU could pull a maximum of a certain amount of wattage.
Each other component could pull a certain maximum of wattage.
The AC circuit that feeds power will have a circuit breaker that provides a safe maximum.
An AC circuit that has its maximum wattage pulled through it will heat up and give voltage fluctuations.
So, you can mention to them that under unpredictable scenarios they'll chance having spurious electrical fluctuations if they chose the 120VAC feed and then push the system with a high data load.
I wouldn't take the chance on data corruption just because they care to not pay an electrician to run a proper power feed.
