I think you can use the WAIT workflow using Duration('P90D') then Send Email, triggering it upon save and upon modification. Make sure that the Re-evaluate Rule Conditions After Wait option is checked so that the previous WAIT would be reset upon modification.

Initially when record is created , created and modified date is same. Create a wf with triger point as When new record is saved, Wf condition is Created Date = Modified Date, Wait for a period of three months or you can use duration function . Check the re evaluate condition and send an email to the owners manager as one of the action.
