I found two reason
1. there are multiple DB connections opened. if this is the case, increase connection pool size using weblogic console or restart your servers along with DB(stop everything and start again).
2. may you are putting the wrong password for xelsysadm user. may some one has changed the password. try login using end user if you know the password. if this is the case you can simple update usr_password of xelsysadm using this known user password in usr table. even update usr_locked=0, usr_loggin_attmpt_ctr(similar)=0 and restart the server. try login using end user password
You can also check startManagedWeblogic.sh for any hard-coded values provided for WLS_USER & WLS_PW ,if you have provided any values, remove it .
