I have 2 select lists, the latter with a cascading LOV option.Now this works fine when selecting an option from list 1, list 2 populates correctly.

My issue is when I re-populate list 1 and list 2 from an onload process only list 1 loads correctly, list 2 does not show the correct value (But it does have the correct value is session).
You have the following in the LOV query of P1_CHILD:
begin select parent_id, child_id into :p1_parent, :p1_child from holder where parent_id = 1; end;
For p1_parent = 1, the LOV query returns 2 rows - DUNDEE and GLASGOW. The select list, therefore, cannot set just one value. I changed the P1_CHILD page item to a multi-select list and it works fine now.
select col2 d, col1 r from child where col3 = :p1_parent
