Did you change any hidden variable in the global(Zero) page. Check the value protection mode.

R u using any hidden item that has value protected and trying to update on page load.
No, all my items are visible.
what ver of APEX r u running. Also is it possible to create it in apex.oracle.com. did you try to debug the page you might get why r u getting the error.
I originally developed the application using APEX 4. The application was working normally. The problem started when the hosting company upgraded the platform to 4.1.1.00.23.
I can not upload the application on apex.oracle.com but it's available on this website www.e-oncologist.com
Based on the research that I did I found the problem is related to the thread below but I'm not sure how to fix that in my case

Check your Display only type item.
Edit your item -> Settings
Change Save Session State to no

I don't think the error is in the display only items. I decided to remove the region with all the items and the website started to work normally, then I added one text item and the problem started again. So if their is only empty region the page is working without a problem but the problem starts again when I add any type of items.

I wasn't able to reproduce the error on the "Contact" page. Instead, the error occurs on your "New" page. I was wondering if you could run the application in debug mode and upload the outputs? Did you also attempt to upgrade your 4.0 app to 4.1?
