ORA-19506: failed to create sequential file, name="string", parms="string"

Cause: call to create the sequential file returned an error

Action: check additional messages, check access permissions

The error you get comes from media manager as it's written in Oracle Documentation: :==
    http://docs.oracle.com/cd/B13789_01/server.101/b10734/rcmtroub.htm#1010097

The above clearly show that there is some issue with your backup server (May be TSM). New sessions are being rejected by the server. You need to check with your ADMIN who is managing your backup servers.
