Kindly check that select account_status from dba_users where username='SYSMAN'
account status should not be locked or expire.
I was able to resolve the issue. The parameter job_queue_processes value was 0, so Grid is not allowing any logins. I changed it to 200 and bounced OMS, then was able to get into grid.
i am having same issue neither ldap authentication is working nor sysman credentials are working in grid 12c. my parameter job_queue_processes in database is set to max 1000 and yet I am having this issue?
Can I please get help in resolving this at earliest?

I tried to login on OEM 12c release 2 cloud control console as other user it show the following error
Authentication failed. If problem persists, contact your system administrator
but when i try with sysman account then it successful done. so i dont know why it happen. I also checked job_queue_processes that is 1000 and i also checked listener,agent,oms status all are up.
so please suggest me what should i do and what is the main cause of this problem.

Verify the following:
1. lsnrctl status
2. Verify the following document:
188.8.131.52 : Login to Console Fails With "Authentication failed. If problem persists, contact your system administrator." (Doc ID 1493151.1)
3. If nothing works, emoms.trc file would help us to understand the error tracked during the login.
Yes Venkat was right.
You should try to look at emoms.trc for the log trail. Mine was corrected when I performed max shrink on the tablespace.
Saw this error and performed max shrink.
