Think of how many people probably use the tool online every day - something like this simply wouldn't be tolerated, and would probably be found by the APEX team themselves.
The toolbar is working fine for me. A likely issue is any browser extensions you might have enabled. Try turning things off, or use a completely different browser.
Either way, before a bug report is logged they're gonna need to be able to reproduce the issue.
No, it is not just me. I provided specific instructions on how to reproduce the issue. Feel free to login to my workspace on apex.oracle.com (vikasa/guest/otnforum) and run page 57688:8. When there is a unhandled exception in show or accept processing and the error page is displayed, the JS functions to launch the Session and View Debug popup/links on the developer toolbar are not included on the page so the links are unusable.

Anyway, now I can log in I see what you mean. Simple workaround is to have the session/debug window opened before landing on that page - something I do all the time.
