If you get forms 4.5 running with DB 11g is very doubtful. At least it isn't certified if that's what you mean. So you should consider moving to a up-to-date version of forms while you are at it...
Base level Forms 6i doesn't work with Oracle 11g, only a couple of the final patch levels allow it to work, and then only in certain circumstances (eg specific database char sets)
So as Christian says, it is very unlikely that you could get it to work.
It is possible but I also recomend the same thing that Christin recomended for you. Its good habit to stay upto date with new technology. May be initially it feels lot of burden to your company but you feel happy with in short period with your decesion to mov to 11g Forms.

Yes correct. It is good only to go for upto date with new technology. But my client asking the documentation for not supported with this version. Can you provide any Doc's or meta links?
Well, the fact that forms 4.5 is desupported for some time should be reason enough. I guess there is no certification information available anymore for forms 4.5; but if memory serves even 10.1.2.0.2 is not certified with Database 11.2.x (you'll have to upgrade to 10.1.2.3), so I guess you can derive that all lower versions aren't certified as well.
