My EBS version is 11.5.10 (DB version 188.8.131.52), most of the doc mention the application as 184.108.40.206, do I need to move to Application version 220.127.116.11 ??? If need then let me know the Doc no.You already have the docs referenced above -- Go with "Path A: Standard Upgrade Path".
Then I will upgrade DB to 18.104.22.168 and then 11.2.x
Then Application from 22.214.171.124 ---> 12.1.1

I was going through the below dos and bit confused...Since you can't upgrade directly from 126.96.36.199 to 11gR2, you can either upgrade to 188.8.131.52 then to 11gR2 (184.108.40.206) or upgrade to 10gR2 then to 11gR2 (220.127.116.11) -- Complete Checklist for Manual Upgrades to 11gR2 [ID 837570.1]

Doc 216550.1 says to upgrade the database to 18.104.22.168
and doc 761570.1(Path A) says to upgrade database to 10.2.0
Could you please let me know how to go about it (for e.g. First database upgrade to version x.x.x.x.x and application to 12.1.1)...

I would suggest you to upgrade your DB from 9i to 10gR2 and then you can move ahead towards 11gr2 upgradation.
For upgradation of 9i to 10gR2 please refer below doc.
