Thank you for bringing this issue to our attention. I've logged bug 13794617 to track this issue, and we have made a Patch Set Exception (PSE) available for download on https://support.oracle.com. Please log in to https://support.oracle.com, select the Patches & Updates tab, and in the Patch Search region to the right of the Patches & Updates page, search for patch *13794617*. Download the patch zipfile, and ensure that you read the readme.txt file contained within it before running the catpatch.sql script against your 4.1.1 instance.

I also encountered the same issue when converting an Oracle Form to APEX. Our DBA says our Oracle database is 11203 but the patch you provided is for database 11202. Is it also applicable for database 11203? Or is there another patch for 11203? Thanks!
