Yes you need to apply the pre-req patches listed, these are mandatory requirements
Per note 393931.1 "Deploying Sun JRE (Native Plug-in) for Windows Clients in Oracle E-Business Suite Release 12" the requirement is listed as:
"Oracle forms 10.1.2.3 or higher with Patch 14614795 or later (See Document 437878.1 titled, 'Upgrading OracleAS 10g Forms and Reports in Oracle E-Business Suite Release 12' for the latest available forms patches)"
So you can apply the patches listed in Note 437878.1 as an alternative, which will also get you onto all the latest code, so this may be the prefered option for you

so , as i am already on Forms 10.1 (Form Compiler) Version 10.1.2.3.0 (Production) , from the below content copied from 437878.1 , it does mean like , i can directly , apply bundle patch 14825718 , instead of the patches mentioned in the Step 2.1.2 of the same doc , true is it ?
Note: Several Forms 10.1.2.3.0 fixes have been consolidated in bundle patch 14825718. Customers are strongly recommended to uptake the bundle patch documented in Step 2.1.2. If customers are already on 10.1.2.3.0, follow instructions in Step 3.2 to apply the bundle patch.

When I did this, I actually rolled back 9593176 before applying 14825718. In the note Upgrading OracleAS 10g Forms and Reports in Oracle E-Business Suite Release 12 [ID 437878.1] following is written under section 3.2 as a small note:
"....Customers who have applied overlay patches 14577216, 14262118 or 14614795 on previous bundle patch 9593176, must rollback the overlay patches explicitly before applying the latest forms bundle patch 14825718...."
By the way, for me the verification steps after 4.5 (verify the upgrade via help) failed because the forms would not come up. I had to apply patch 12965674 and run "txkrun.pl -script=CfgOC4JApp" to overcome that issue.
