For the first one I need more information.
for the second one you have to schedule your test in OTM and add an email or emails to Edit Task Section (Send report to). This will send the report after the test is executed (passed or failed test).

For the first one, Generally I know the option to create issue is go to issues Tab --> Create issue manually --> attach any screenshot for failure etc... and one is is created we will associate this to the test case. My question is is there any way where we can create/generate the issue at the step where the test script failed in OTM
